How bureaucracies naturally accumulate “code debt”: changes to a bureaucracy require emotional activation. Sufficient emotional activation only occurs around acute negative events. Not around the small costs that add up of having lots of rules or rules which no longer make sense. So we naturally get buildup. And there’s no counterbalancing mechanism.
